   1. Here you get a chance to add your information: Special requests or comments (optional)

   2. Cancellation Policy: All appointment cancellations must be made 24 hours prior to your scheduled appointment time. Any late cancellations or missed appointments will result in a $100 charge to the credit card on file.

      3.  Click the Book Appointment Button. Make sure to pay before your session. If payment is not made then your spot will be allowed to be booked by someone else!

      4. After Completed: 

         1. You will receive your Consent Form via Text / Email after. You will need to bring your Photo ID for your appointment. This must be filled out before your appointment. ​

         2. You will also receive a booking form so we can make sure your appointment goes as smoothly as possible and make any corrections prior to your appointment.
